IASCCI understands the Senior Care Consultant is a knowledgeable specialist who assesses, develops, coordinates, implements and evaluates an individual plan of care for the elderly person.

The Registered Nurse or Licensed Clinical Social Worker working as a Senior Care Consultant will provide a comprehensive assessment of the elderly person’s medical and psychosocial status in order to identify current areas of need and will collaborate with the older person, their family and medical and associated providers as appropriate. The assessment will identify options and solutions for appropriate care or assistance.
